
Parent Involvement in Sports
Parents play many important roles in sport — as current or former athletes, coaches, fans, motivators, role models, and sometimes even critics. Their influence reaches far beyond the sidelines. Research shows that family members may shape an athlete’s involvement, confidence, and long-term achievement in sports even more than coaches do.
Parents are often the first and most impactful agents in socializing children into sports. They introduce the game, model attitudes toward competition and teamwork, and help shape how young athletes define success — whether it’s about winning, improving skills, or simply having fun.

Forms for Your Athlete's Season
Here you’ll find all the necessary forms for your kiddo’s volleyball season. If you have any questions, please reach out to us anytime!
Once your athlete accepts a written offer from ULVBC to join a team, we will need the following completed via our Google Form:

USAV Medical Release – Each player must complete this form.

Proof of Age – Provide a copy of a birth certificate, passport, or driver’s license for each player.

Spectator Code of Conduct – Each parent must sign.

Players sign the Player Expectations / Code of Conduct form.

Parents sign the Parent Expectations form.

Important: All completed documents must be emailed to upperleftvolleyball@gmail.com. Do NOT give these documents to your athlete’s coach.
Costs & Program Details
Club Association Fees
Amount: $100 (one-time fee)
Due: As soon as you sign the acceptance letter.
Note: This fee is required for every player.

Payment Options: Program fees can be divided into 3 payments.
Tournaments & Travel

Tournament fees and travel costs are invoiced a few times during the season, not per tournament.

Tournaments: Registration cost is divided among the number of players on the team (e.g., $400 tournament ÷ 10 players = $40 per player).

Travel: Coach travel costs (hotel, transportation, per diem, etc.) are also divided among the number of players.
Uniforms

Players will borrow uniforms at no cost.

Uniforms are issued at the start of the season and must be returned at the end of the season.

Disclaimer: Players and families are responsible for any lost, stolen, or damaged uniforms. Replacement or repair costs will be charged accordingly.
